Swagga Radio's night host, DJ Blaze, aka Dwayne Malcolm, died while on vacation in Jamaica.  He was 34. Dwayne Malcolm is survived by two daughters, Jaylen and Harper Malcolm.  We wanted to wait for a statement from the Malcolm family, and pass along details as we receive the information from the Malcolm family.

From the Malcolm family:

"It is with a heavy heart that I confirm the passing of my brother Dwayne Malcolm "DJ Blaze".  On Wednesday, July 26th our entire family traveled to Jamaica for a Family Reunion and Wedding.  On July 29, 2017 at approximately 6:15am an oncoming motorist tried to overtake a truck and slammed into Dwayne's car.  The accident took the lives of Dwayne and our cousin Jermaine.  It also caused serious injuries to our very close family friend/third passenger Daniel Middleton.  As you can imagine, this tragedy has caused a type of grief that is explainable.  Please continue to keep us in your prayers as we face the days to come.  We are in the process of making funeral arrangements, which will be shared with the public as soon as it  is available. Attached is a link to a GoFundMe account established as a scholarship fund for Dwayne's two daughters Jaylen and Harper Malcolm."

At the request of DJ Blaze's family, we wanted to share this link.

The following is a direct quote from the official GoFundMe page:

On July 29th, the 34-year-old community leader, community service organizer, and kindred soul of Dwayne A Malcolm was taken from earth while attending what was supposed to be the ultimate relaxing getaway and Jamaican independence celebration in the form of a family reunion. The car accident also claimed the life of his cousin, causing a great deal of stress - not only emotionally for the family, but financially as well. This GoFundMe is to establish a scholarship for his beautiful 1- and 13-year-old daughters' future.  Whether large or small, every token received will be with love, and greatly appreciated. One love. One heart.
